How to Get Started

Starting with SmartCare Link is simple and DHS-compliant. We guide you from your first question to your first approved service.

Step 1 – Choose SmartCare Link as Your Consultation Provider
We help you understand your options, compare the Agency Model and Budget Model, and decide what’s best for you.

Step 2 – Receive Your MnCHOICES Assessment
A Certified Assessor from your lead agency will meet with you to identify your needs and determine your eligibility for CFSS. We’ll work alongside them to ensure your plan reflects your goals.

Step 3 – Begin Your CFSS Journey
Once your assessment is complete and your Individual Service Delivery Plan (DHS-6893P) is approved, we help you launch your services—whether that’s hiring workers, purchasing goods, or both.

Partnership for Agencies

SmartCare Link works closely with lead agencies, managed care organizations, and Financial Management Services (FMS) providers to streamline referrals.

How to Refer:
Agencies can submit referrals via our secure portal, encrypted email, or fax. We also accept direct referrals using DHS forms such as DHS-8477A.
